What is the cheapest legal cover?

The Essential Benefits Plan, roughly AED 700 to 1,200 a year for one person. It meets the health authority minimum. It also has a small network and an annual cap that a single hospital stay can reach.

Is maternity included anywhere cheap?

Not in the Essential plan. Adding it starts around AED 4,000 and almost always carries a waiting period, so it has to be bought well before it is needed.

Does the employer have to pay?

The employer must cover the employee. Family members are usually your own cost, and that is where the bill grows.
